养老险理赔申请材料

4.门诊病历 Out-patient case history
5.住院病历或出院小结 Hospital case history or Inpatient summary
6.医疗费用明细清单/处方 Medical expense details /prescription
7.病理/血液/影像检查报告 Pathology/Blood/Image report
8.居民死亡医学证明书或法医鉴定书 Medical certificates of resident’s death or forensic identification
9.户口注销证明 Certification of Household registration write-off
10.丧葬火化证明 Funeral and cremation certification
11.有关部门出具的意外事故证明 Accident certification presented by relevant departments
12.残疾鉴定报告 Report of disability identification
13.受益人身份证明 Identity of beneficiary
14.受益人与事故者关系证明 Certification of relationship between the beneficiary and the Insured

1.受益人的指定和变更Designation and alteration of the beneficiary
(1).身故保险金Death Benefit
被保险人可指定一人或数人为受益人，受益人为数人时，应确定受益人顺序和受益份额，未确定份额的，各受益人按照相等份额享有受益权。投保人经过被保险人书面同意后可以变更受益人，但需书面通知本公司，由本公司在保险单上批注。
The insured can designate one or more people as the beneficiary. If there is more than one beneficiary, the sequence and the share of the beneficiary should be confirmed. If not confirmed, each beneficiary will take the benefit with an equal share. After notifying our company in written form, the applicant can change the beneficiary based on the written approval from the insured, and our company will make endorsements on the policy.
未指定受益人的，则由本公司依据《中华人民共和国继承法》的规定向被保险人的法定继承人履行给付保险金义务。
If no beneficiary are designated, our company will fulfill the obligation of the benefit payment to the heir at law according to Inheritance Law of the People’s Republic of China.
(2).生存保险金 Survival Benefit
医疗保险金、意外残疾保险金、重大疾病保险金等生存保险金的受益人为被保险人本人，本公司不受理指定或变更。
The beneficiary of survival benefits for medical expense, accidental disability and dread disease, etc.) must be the insured himself/herself. Our company will not accept any designation or the alteration.
2、申请人为被保险人、受益人或其监护人，如受益人为多人时，请另外填写《理赔申请资格确认表》，如申请人委托他人办理的，需提供全体受益人签署的《授权委托书》。
The applicant should be the insured, the beneficiary and his/her legal guardian. If there are more than one beneficiary, please otherwise fill in Qualification Confirmation Form on Insurance Claim Applicants; if the applicant entrusts others to handle this, he/she shall provide Letter of Authorization signed by all of the beneficiaries.
3、依照保险法之规定，理赔申请人有义务真实地提供与确认事故的性质、原因、损失程度等相关的证明和资料。若伪造、变造与保险事故有关的证明、资料和其他证据，您的权益将会受到影响；情节严重的须负相应的法律责任。
According to the Insurance Law, the claim applicant has the obligation to provide related true certificates and documents to find out the case nature, cause and loss extent, etc.. If the provided certificates and documents are forged or modified, your rights and interests will be affected; for serious situations, you should bear corresponding legal responsibility.
4、若上述资料尚不足以证明有关情况，本公司可要求您继续提供相关理赔申请资料，以便更好地维护您的权益。
If the documents above are not enough to prove relevant situations, our insurance company could request you to continue providing relevant claim application documents to better protect your rights and interests.
5、若您的理赔申请事项经审核属保险责任范围，本公司会及时通知您领取保险金；为保证保险金受领的安全性，请您尽可能亲自前来本公司领取或办理银行转帐；委托他人领取时，请您完善委托手续，必要时还须公证。
If your claim application items are within the coverage after checking, our company will inform you of receiving the benefit in time; in order to guarantee the security of the benefit payment, please try your best to come to our company to receive the benefit or make bank transfer by yourself; if you commit others to receive the benefit, please complete the procedure of commission, and the notarization is needed if necessary.
